NFL FLAG offers free supplemental participant accident insurance through Players Health for all registered players.

If you already use Matrix to build your teams (Matrix Team Builder) or place your equipment orders through Matrix, your players are automatically opted into this coverage — no further action needed.

If you place a bulk order only and don't use Matrix for team building, you'll need to upload your player data separately to opt into this free coverage. Use the simplified template below — it requires less information than a full Matrix upload, since it's only used to enroll players in the accident insurance, not to process an order.
